About The Chartis Foundation

Supporting primary care for underserved communities

Our mission 

Established in 2006, The Chartis Foundation provides financial support to non-profit organizations dedicated to delivering essential primary care services to underserved populations across the United States. 

We are committed to addressing systemic health disparities and barriers to care faced by communities.

Our support

The Foundation delivers on its mission by offering two types of grants:

  1. One-Time Grant: The Foundation awards one-time grants given for a wide scope of activities for the purpose of supporting primary care services delivery. Examples of past funding include the acquisition of equipment, furnishings, supplies, pharmaceuticals, construction, information system purchases and training.
  2. Fellows Program Grant: The Fellows program grant is renewable for up to three years, subject to performance. This grant supports direct primary care services by sponsoring caregiver and supporting positions at non-profit direct care organizations. Fellows have historically included RNs, NPs, LCSWs, medical social workers, outreach workers and aides. Grant funding may be used either to help fund an individual worker or a team.

Each organization may only apply for one grant per application cycle offered by The Chartis Foundation.
 

Our relationship with Chartis

The Chartis Foundation is funded by Chartis and operates under the guidance of the original founders and Board of Directors, Ken Graboys, Ethan Arnold, and Chris Regan and is managed by Executive Director Jim Kagen. A dedicated volunteer team of Chartis colleagues plays a crucial role in evaluating grant applications.

Through the years, The Chartis Foundation has been critical to Care Ring's ability to serve our community's most vulnerable neighbors. We deeply appreciate their trust-based philanthropy, which allows our staff to confidently deliver patient care. We're grateful for their partnership in extending care to those who need it most, and for their dedication to our shared values, especially compassion.
